Eligibility: - Report must be made by phone and then followed up with a written report submitted via fax (see attached Report of Suspected Dependent Adult/Elder Abuse form); Reporters: All ages; Elders: 65 and older; Dependent Adults: 18-64 years, who are physically, mentally or developmentally disabled; Also included is anyone of this age who is admitted as an inpatient to a 24-hour facility
