Aims to give people more complete health care and help with health-related social needs. Services by a Care Manager may include: Case management Coordinating health care with doctors, dentists, optometrists, and specialists Help with understanding prescriptions Help arranging medical transportation Help with first month's rent when moving into a new place. Serves people with disabilities. May also include those who are: Those at risk of homelessness Unhoused adults wanting to transition into permanent housing Adults transitioning from a nursing facility or assisted living facility back to a home environment Adults transitioning from a correctional facility to community living Youth enrolled in CCS (California Children's Services) program Youth involved in the welfare system (foster care).
