Provides comprehensive care management for people with disabilities. Must have Partnership HealthPlan. Aims to give people more complete health care and help with health-related social needs.
Services by a Care Manager may include:
- Case management
- Coordinating health care with doctors, dentists, optometrists, and specialists
- Help with understanding prescriptions
- Help arranging medical transportation
- Help with first month's rent when moving into a new place.
Serves people with disabilities. May also include those who are:
- Those at risk of homelessness
- Unhoused adults wanting to transition into permanent housing
- Adults transitioning from a nursing facility or assisted living facility back to a home environment
- Adults transitioning from a correctional facility to community living
- Youth enrolled in CCS (California Children's Services) program
- Youth involved in the welfare system (foster care).
