Description

Provides youth transition program for people with disabilities ages 14 to 30. Offers the following services: - Tools, and support to transition from school-based services to independent living
- Information and referrals
- Guidance with center and community services.
One may also have the opportunity to qualify for a paid Peer Mentor Internship. Services help one with: - Education planning - connects to local education programs and resources
- Exploring a variety of job fields and finding out if one has what it takes to do the job
- Being more independent - teaches skills like budgeting, healthy eating, and personal organizing
- Improving social skills - explores how to make new friends in the community and at one's job.
