Submit an application in-person, by mail, or online through a third party vendor called www.vitalchek.com (vendor does charge additional fees); No appointment necessary for the public to walk-in and purchase certificates in-person; To register a birth that took place at home or outside of a hospital setting, please call the office to schedule an appointment to register the birth

Completed application for a Vital Record (application can be found on the website), Government-Issued Picture ID; If mailing an application, a Sworn Statement on the second page of the application must be notarized by a Notary Public
