Description
Reaches out to and assists at-risk older adults and their family caregivers in times of crisis and emergencies. Recipients receive:
  • Wellness check by phone for support, along with risk and needs assessment
  • Care planning
  • Coordination of necessary support, services, and supplies delivery.
Aims to reach rural individuals and families who otherwise might not be helped, in order to prevent further harm and maintain wellness. This program is made possible with funding from United Way of Northern California.
