Description
Provides housing navigation and transition support services for people with disabilities. Must have Partnership HealthPlan. Services include:

Housing Assistance:
- Housing Transition Navigation Services to help individuals secure stable housing. Services include:
- Help with locating housing possibilities
- Affordable housing list
- Help with filling out HUD (Housing and Urban Development) and other housing applications
- Help with filling out and paying for housing applications.
- Financial support for housing deposits or rent payment assistance for those who qualify
- Housing Tenancy and Sustaining Services to promote long-term housing stability.
- Advocates with neighbors and/or landlords, as needed, if they are not understanding ADA and disability rights.
- Help with creating a budget to help one know how much they can afford.
Serves people with disabilities who are experiencing or at risk of homelessness. May include those who are:
- Unhoused adults wanting to transition into permanent housing
- Adults transitioning from a correctional facility to community living
- Youth enrolled in CCS (California Children's Services) programs
- Youth involved in the welfare system (foster care).
Note: This program does not directly provide housing or help with paying utilities. However, upon approval from Partnership HealthPlan, they can pay a utility deposit and/or pay a back bill. This is only for clients they help transition into permanent housing.
