Provides comprehensive care management for Partnership HealthPlan clients. Works to give people more complete health care and help with health-related social needs. Services by a Care Manager may include:

  • Case management
  • Coordinating health care with doctors, dentists, optometrists, and specialists
  • Coordinating care for mental health services
  • Help with understanding prescriptions
  • Arranging medical transportation
  • Social services
  • Homelessness resources and more.

Also offers Community Supports. Assists clients in applying for help with:

  • Medically Tailored Meals (temporary home delivered meals for those with a qualifying chronic condition, or at risk of hospitalization)

  • In-center respite care.
