Description
Dignity Health Connecting Living provides comprehensive care management for Partnership HealthPlan clients. Works to give people more complete health care and help with health-related social needs. Services by a Care Manager may include:
- Case management
- Coordinating health care with doctors, dentists, optometrists, and specialists
- Coordinating care for mental health services
- Help with understanding prescriptions
- Arranging medical transportation
- Social services
- Homelessness resources and more.
Also offers Community Supports. Assists clients in applying for help with:
