Provides youth transition program for people with disabilities ages 14 to 30. Offers the following services:

• Tools, and support to transition from school-based services to independent living
• Information and referrals
• Guidance with center and community services.

One may also have the opportunity to qualify for a paid Peer Mentor Internship. Services help one with:

• Education planning - connects to local education programs and resources
• Exploring a variety of job fields and finding out if one has what it takes to do the job
• Being more independent - teaches skills like budgeting, healthy eating, and personal organizing
• Improving social skills - explores how to make new friends in the community and at one's job.
