Description

Dignity Health Connecting Living provides a protective supervised day program for individuals with medical conditions.

Services include:

  • Personal care services, protective supervision, and/or help in activities of daily living
  • Therapeutic activities tailored to the individual's abilities
  • Nutrition services
  • Basic health monitoring
  • Transportation coordination
  • Respite and support for families
  • Physical and occupational therapy
  • Medical services.

Program Delivery
Eligibility: - Needs Medi-cal/Partnership & diagnosis that leads to difficulties in ADL's. Or VA healthcare doctor can refer. Or Cash for $140/day.
-Open to adults ages 18 and up with disabilities
-Also serves those at risk of requiring nursing home care
Languages: Spanish, English
