Provides preventive, acute, chronic health, and behavioral health services. Serves those who are economically or otherwise disadvantaged. Offers a continuum of care from prenatal to adulthood.

Health services include:

  • Primary care
  • Pediatrics
  • Telehealth appointments
  • Adult medical exams
  • Gender health services
  • Diabetic education
  • Employment, sports, school physicals
  • Family planning services
  • Foot care
  • Immunization services
  • Maternity services
  • Women’s health exams (paps, breast screening, etc.)
  • On-site pharmacy, lab, and x-ray services
  • Tattoo removal. Must be gang and/or human trafficking related.
  • Entry point for the Coordinated Entry System (CES). Links clients to a variety of housing assistance services. These can include rapid re-housing and permanent supportive housing.

SCHC offers After Hours Clinician Access for established patients. To reach a clinician after hours, call 530-246-5710 and press 1 for the Nurse Advice Line.
