Eligibility:

-Open to patients of Shasta Community Health Center (all ages)
-Must have a referral from an SCHC clinician for services

Languages:

Translation services available for most languages; Must notify in advance to set up American Sign Language, Mien, Lao, and Spanish, Laotian, English
|

Application Process:
|
Need SCHC physician referral; if not a current SCHC patient, one may call one of the SCHC centers to be added to their waitlist, or see a clinician at the HOPE Van for a quicker referral; Patient must establish care with SCHC, cannot have two primary care clinicians
